Accessing Services
Individuals interested in seeking addiction treatment can call the Central Intake and Case Management Department at 1-844-621-4080, or by walking into any of our outpatient clinic locations.
When you call or come in to our office, your process usually begins with a brief screening to gather some necessary information. If a person is determined to be in urgent need at the time of the screening, he or she will be assisted to access immediate care. If there is no emergent need for care, you will be given an appointment for a more in-depth interview, called an assessment, with one of our case managers. After your assessment, your case manager will refer you to the appropriate level of care. A case manager may also make a referral for additional supports such as Recovery Supports and Case Coordination.
Case Coordination
If at any time during an individual’s treatment episode, a non-treatment need is identified, case coordination services will be offered. Our Case Coordinators partner with our local MAT providers to coordinate medicated assisted treatment care. Case coordination is a service component of case management that assists those who may lack the resources to meet identified needs, such as:
- Housing
- Physical Health
- Child Care
- Basic Needs
- Legal Services
- Educational
- Mental Health
- Transportation
- Employment
- Vocational Training
A Case Coordinator will work with you to identify and set personal goals and to access services as you work toward achieving those goals, providing encouragement and advocacy along the way.
