The B-MAT Program

A Collaborative Effort Between

Bradford Regional Medical Center

Alcohol & Drug Abuse Services, Inc. (ADAS, Inc.)

And

Potter County Drug and Alcohol Program

THE B-MAT PROGRAM

This program is a multi-discipline, collaborative team effort bringing together prescribing physicians, substance use therapy, case coordination and recovery programs for the treatment of opioid dependence with Buprenorphine.  The program begins with an assessment by ADAS, Inc. or Potter County Drug and Alcohol Program to identify consumers to be referred to the physician and the psychotherapy services.  The goal of the program is to promote recovery with collaborative services all while providing individualized care and commitment to the consumer.

NETWORK AND REFERRAL

Any team member contacted for buprenorphine services.

Referral to ADAS Case Management/Coordination if C.E.M. Co. resident

Or

Referral to Potter County Drug and Alcohol Program if Potter Co. resident

D&A Assessment completed by above, if B-MAT admission criteria met, the program model and consumer agreement are reviewed.  If admission criteria are not met, continue as current operating procedure.

Referral to ADAS for psychotherapy services (continue as indicated and regulated)

AND

Referral to Physician services at BRMC (preferably the next Thursday)

Physician evaluation, nurse intake and evaluation and urine sample collection (first Thursday)

Initiate Prior Authorization for Medication

Physician inducted visit, urine sample collected (Following Thursday)
